Non-clinical Errors Can Also Cause Patient Harm

As part of an online safety culture course, I developed a brief animated video focusing on an incident that took place at Duke University Health System. The video highlights that medical errors can happen beyond clinical environments and involve non-clinical staff members.

Development Process

To engage the learners, I devised a plan to commence the video by providing a short overview of the medical error and the subsequent need to send letters to 3,800 patients. A detailed explanation of the error would then follow the intro. While sketching each event on paper to create a timeline, I had the idea of transforming the video into a visual representation of the timeline. This visual effectively emphasizes the duration between the initial error and its eventual reporting.

Afterwards, I developed a basic script and created storyboards for each timeline section, ensuring they matched the script's timing.

Using Adobe Illustrator, I designed the graphic for the storyline and imported it into After Effects for animation. Then, I utilized Illustrator to create the graphics and animated characters before again using After Effects to animate each scene. Finally, I incorporated the animated videos and B-roll footage into the animated timeline.

Once the animation was finalized, I recorded the voiceover using Adobe Audition. I then brought the animation and voiceover files into Adobe Premiere for final adjustments and alignment.
